Company Profile

1. Provide information on the registered name and address of your company: Input Company's RC Number)
2. Company overview - history, years in business, major shareholders, etc, and shareholding structure (i.e. private or public company) with the names of your main shareholders and subsidiaries/partners

Please provide your service teams/structures to support this Project. Include the resume of the key team members: Professionals are to include -

1. MBBS & BDS with Fellowship of the National PostGraduate Medical College/West African Postgraduate Medical College
2. Minimum of 5 years Post Fellowship experience
3. MSc/PhD in Medical & Dental Public Health

• Please state the name of any third party which will be delivering services and on-site support for the services on your behalf
• If any part of the service has been/will be outsourced to another firm, how long has this relationship been in effect?

Scope of Requirement

Develop a comprehensive brief for execution that covers areas highlighted in the SoW that key to siting a sustainable, accessible and befitting health facility that will be affordable and standardized. Please note the pillars in the SoW - sustainability, partnerships, technology, governance etc.

• Identify Risks & Compliance issues that may impact this Project and mitigation plans
• Develop a high-level work breakdown structure (program) in line with stated plan of execution
• Other additional deliverables that will be beneficial to the scope of work

Industry and Geographic Track Record

Provide the details (names of health facility, location, project duration, cost, etc) of similar projects carried out by your Firm over the last 3 to 10 years in Nigeria and outside Nigeria

What sustainability measures were put in place in the previous projects undertaken
